Date(s) to observe: * First and last name: * Email address: * The identifier or screen name that you will be using when joining the virtual hearing ("Screen Name Identifier"): * Observer Rules and Expectations Entry into the proceedings will only occur before the hearing begins or at break times. Only the list of confirmed Observers will be allowed entry. Observers must join the Hearing 5-10 minutes before the hearing’s scheduled start time. Observers must remain silent at all times and are asked to mute their connections. Observers must ensure that they have, and know how to use, the required video conferencing platform for any virtual hearing, which will be communicated in advance of the hearing. Technical troubleshooting will not be provided to Observers at a hearing. Observers must display their Screen Name Identifier along with “Public Observer” as their screen name so that the Hearings Director can identify them and allow them into the hearing. In some cases, an Observer's email address, as supplied above, in addition to the Screen Name Identifier may be visible during the hearing. The Alberta College of Dental Hygienists will use its best efforts to minimize visibility of email addresses but may be limited in doing so. Observers are required to have their screen camera turned off, unless specified by the Chair otherwise, and position themselves in a quiet, private space with minimal background movement. Observers may be asked to leave the hearing if their video presence is causing distraction. Observers are not permitted to, or cause others to, record, photograph, rebroadcast or otherwise recapture, in any manner, any portion of the proceedings and must undertake (by the registration process) to not do so. The chairperson of the Hearing Tribunal is responsible to maintain order during the proceedings and, as such, has the authority to ask an Observer to leave. Participants at the hearing, including the Hearing Tribunal members, conduct counsel, the investigated person, or their legal counsel or support persons, are not available to answer questions from Observers before or during the breaks. If the chairperson orders the entire hearing or portions of the hearing to be held in-camera (private meeting), the Observers will be asked and must leave the hearing. I, the above named, have read the above, understand and so undertake to abide. * Draw Type Clear Today's date * Leave Blank This site is protected by reCAPTCHA and the Google Privacy Policy and Terms of Service apply.
